Procedure to Apply for a Licence to Operate a Private Home/Hospital
- Complete the Application Form. CLICK HERE to Download the Application Form.
- Attach a floor plan of the building, showing the location and use of each room and its dimensions.
- The building/home to be used must be inspected and approved by the Fire Service Department and issued a Trinidad and Tobago Fire Services Fire/Life Safety Certificate
- Submit evidence of payment of licence fee, along with Application Form to the Office of the Chief Medical Officer, Ministry of Health, #4 - 6 Queen's Park East, Port of Spain.
