Agent Authorization Letter
To be completed and signed by a director of the Applicant Company identifying its agent authorized to conduct business on its behalf.
Tobacco Application Checklist
The Applicant is to provide the required documents listed in the checklist as relevant to the applying company. ( Manufacturer, Distributor, Importer, Exporter, Wholesaler).
Statement of Declaration
Is required to be filled out by a director of the applicant company which confirms the correctness of the information provided in the application and compliance with the Tobacco Control Act and its Regulations as amended.
